I am in-network with BlueCross and BlueShield (BCBS) PPO of Illinois. I can also provide Superbills for out-of-network reimbursement.

Self-Pay

I offer a free 15 minute consultation session to determine if we are a good fit. Our initial session, also known as a diagnostic evaluation, is $190 and 60 minutes long. After the initial diagnostic evaluation, all ongoing sessions are $170.

All fees, including co-payments, co-insurance fees, and self-pay fees, are due at the time of service
